SLDM Colleges of Quezon City – Admission Overview
Senior High School Admission
Senior High School (SHS) Applicants: SLDM QC admits incoming Grade 11 students (all tracks) and transferees (Grade 11 or 12) from other schools. It also welcomes Alternative Learning System (ALS) graduates (Category E voucher applicants) with a TESDA Form 5 (A&E accreditation). As a private SHS, SLDM participates in DepEd’s voucher program: NCR voucher values are ₱18,000 for most students and ₱22,500 for ALS/PEPT passers. Applicants must complete a SHS Application Form and submit their documents to the Admissions Office.

Eligibility & Special Cases: Open to Grade 10 completers (private or public) who have not yet taken Grade 11, as well as transferees. ALS passers are eligible but must present a certified ALS Form 5 and Certificate of Rating (for SHS Grade 10 A&E). Transferees must provide an Honorable Dismissal (Certificate of Transfer) from their previous high school, including recent grades.

General Requirements: Documents typically include a PSA/NSO birth certificate, original Form 137/Form 138 or TOR (for private HS) showing Grade 10 completion, updated 2×2 ID photos, and a good moral character certificate. Female applicants who are married should also submit a marriage certificate. Additional clearances often requested are a barangay residency certificate, recent medical/health certificate, and (for voucher applicants) proof of income (notarized affidavit).

Application Process: Applicants fill out the SHS application and pay any processing fee. They then submit all requirements (as listed above) to the Admissions Office. SLDM may require an interview or entrance test, but admission is primarily based on complete documents. Accepted students are given an enrollment schedule. (Voucher applicants must apply online via PEAC’s portal and then enroll at SLDM upon approval.)

Voucher/Scholarship Notes: SLDM is a voucher-eligible private SHS. Incoming Grade 11s should register for the SHS Voucher Program (through PEAC/OVAP); Category C and D recipients receive ₱18,000 (NCR rate), while ALS/PEPT qualifiers (Cat E/F) receive ₱22,500. The school assists qualified students in redeeming vouchers against tuition and fees.

Technology Vocation Admission
TESDA NC II Program Applicants: SLDM’s TESDA-accredited vocational courses (e.g. Computer Systems Servicing NC II, Contact Center Services NC II, Housekeeping NC II, Visual Graphic Design NC III, Web Development NC III, etc.) have separate requirements. Admission is generally open to out-of-school youth and adults.

Basic Qualifications: Applicants must be at least 18 years old and have completed secondary education (high school graduate or ALS A&E Certificate). All trainees must meet TESDA’s entry rules: age 18+, HS diploma (or ALS Form 5 for non-grads), and willingness to take any required pre-training assessment.

Required Documents: Typically, submit clear copies of PSA birth certificate, high school diploma or TOR (or ALS Form 5 with Certificate of Rating for those who took A&E), and 2×2 or 1×1 ID photos. Other standard items include a barangay clearance, a medical/physical exam certificate, and a good moral character certificate. Female applicants should include a marriage contract if married. Some courses may require NBI/police clearance (especially for hospitality/security courses) and National Career Assessment Examination (NCAE) or equivalent.

Special Cases: Generally, there are no ALS or transferee-specific rules for TESDA courses beyond the HS/ALS certificates. Non-graduates must clear the ALS A&E or PEPT, as appropriate (ALS passers present Form 5 and CoR). Transferring between TESDA courses simply requires the student to settle any balances and apply anew.

Application Steps: Interested students should inquire at SLDM’s TESDA Training & Assessment Center (phone numbers or in-person). They must complete a TESDA enrollment form and submit all required documents. SLDM may administer a short assessment or orientation. Once qualified, the applicant reserves a slot. Courses may have rolling enrollment or fixed batches.

TESDA Scholarship/Voucher Slots: SLDM often offers scholarship slots under TESDA’s programs. For example, SLDM QC has been allocated limited Training for Work Scholarship Program (TWSP) slots for CSS NC II and CCS NC II (1 slot each in FY2022). It may also run programs under Special Training for Employment (STEP). Applicants should ask if TESDA scholarships/allowances are available for their chosen course. (Even if not under scholarship, TESDA courses are relatively low-cost and may include allowances for some trainees.)
Additional Notes: All applicants to SLDM should prepare multiple copies of documents. Requirements lists and application forms can be obtained at the SLDM QC Admissions Office. Keep extra copies of ID photos (with name tags) and any clearances. Applicants are advised to contact SLDM directly for application deadlines and any course-specific prerequisites.
